Patient:
Mrs. Parvathamma
A 53 year old female had hypertension for 4 years and
chest pain for 1 week. Due to ECG changes, she was being treated
for ischaemic heart disease for last 4 years. Coronary angiogram
done showed normal coronary arteries and normal LV function and
she was discharged only on antihypertensive medications and no
antianginal treatment. Patient: Mr. Basavalinge Gowda
A 74 year old male came with hypertension, COPD and past h/o
Ca.Larynx. He was admitted with chest pain for 24 hrs. ECG showed
acute inferior wall MI with lateral wall ischaemia. He was thrombolysed
and an angiogram done after 48 hrs showed triple vessel coronary
artery disease - 100% LAD occlusion and 80% lesions in
large OM branch and Mid RCA. He continued to have post infarction
angina and underwent successful bypass surgery after about 4
weeks. He had LIMA graft to LAD Artery and Venous graft to Diagonal,
OM & RCA. He was discharged from the hospital after 6 days.
Patient: Mr. Simon Nazareth
A 53 year old male came to the hospital with chest pain and
both arm pain for one and a half days. He had no risk factors
like high blood pressure, diabetes mellitus or smoking.
His ECG on admission showed an evolving anteroseptal myocardial
infarction with ventricular ectopic beats. Due to his late presentation,
he was not thrombolysed and was treated with IV Heparin infusion
for 48 hrs and two antiplatelet agents - Aspirin &
Clopidogrel with other antianginal medications. He was found
to have diabetes mellitus and severe dyslipidaemia and was started
on insulin, rosuvastatin and gemfibrozil.
His coronary angiogram was done after 2 days which showed two
vessel coronary artery disease - occluded mid LAD artery,
stenosis in large second diagnol and large OM II branches. He
underwent a two stage angioplasty and stent implantation of
LAD, Diagonal and circumflex OM branches with intracoronary
Reopro injection. Post angioplasty he made an uneventful recovery
and was discharged from the hospital after 4 days.
Patient: Mr. Chandrashekar Rao
A 56 year old male came to the hospital with severe
chest pain for about 15 hrs with sweating and vomiting.
His ECG on admission showed ST elevation in leads V1-V4. He
had no risk factors for ischaemic heart disease. He was taken
up for emergency angiogram and primary angioplasty to mid LAD
artery. IV Integrilin Bolus and infusion was given during the
primary angioplasty. He made a good recovery post angioplasty
and was shifted out of ICU after 24 hrs. |
